This form is to register students for faith formation at Most Blessed Sacrament Catholic Church for
the 2024-2025 Academic Year. This form allows you to select one of the following programs to
register your child for:

1.) Parish School of Religion (grades 1-10 for non-catholic school students)
2.) 2nd Grade Sacramental Preparation Programs (for both non-MBS and MBS School Students)
3.) Confirmation (11th or 12th grade)
4.) RCIC- Right of Christian Initiation for Children (for students from 3rd grade to 12th grade
seeking baptism or other sacraments of initiation)

		IMPORTANT:
		This Form utilizes our Parish Data System (PDS) records. If you are a member of Most Blessed Sacrament Parish, you may be asked to enter your MBS Family ID Number for record keeping purposes. If you do not know your MBS Family ID Number, please call the parish office for assistance at (225) 752-6230. If you are a new family to MBS or currently a member of a different parish, select "New Family".


		FOR CONFIRMATION AND 2ND GRADE STUDENTS ONLY:
		- The 2nd Grade PSR option (for non-Catholic school students) automatically enrolls you in BOTH our PSR program and our Sacramental Preparation Program (for 1st Reconciliation and 1st Eucharist). If your child will NOT be enrolled at the MBS School for the 24-25 school year, please select this option for your 2nd grade student. (Please make sure you download a copy of both the PSR and the sacramental prep calendars from the church website for student/parent formation session info).
		- 2nd Grade MBS School Students: please select your class (2A or 2B) for Sacramental Prep. (Please make sure you download a copy of the sacramental prep calendar on the website for student/parent formation session info).
		- Confirmation Students: Please make sure we have a working email for EACH CONFIRMATION PREP STUDENT.


		HELP NEEDED: We are looking for volunteers to fill the following roles for the upcoming year:
		- 2nd Grade PSR Catechist Aide/Assistant (2)
		- 7th Grade PSR Catechist Aide/Assistant
		- 10th Grade PSR Catechist Aide/Assistant


		VOLUNTEER NOTE: Training is available for all positions. Volunteering for one of these above positions allows your student(s) to attend our program this year free of charge (full refund). These hours also will count towards volunteer hours for the MBS School for the 2024-2025 Academic Year. Please help us spread the word about this opportunity to serve your MBS community!
